Yes I have once and it was really good - they do not go whiter than they ever were but mine went several shades lighter.

I had the special holders made at the dentist to fit my mouth so I just buy the vials of bleach - quite easy -just put it on for 30 minutes twice a day or some such depending on the strength.

No. My dentist isn't really into cosmetic dentistry. He just cleans the teeth, fills them, and repairs them. Anything else, he will send you away to someone who specialises in that. When I was on a cruise ship, I saw it on offer at the wellness centre, but it was thousands of dollars per session, as it was probably permanent whitening. I have never smoked, so my teeth look okay, and don't really need it.
the only way to get permanent whitening is to put something over each tooth - otherwise the bleaching will eventually wear off. My dentist was always anti whitening till lately so I figured the one he recommends is OK!
Yes, the whitening a la Shane Warne is permanent isn't it. He has some kind of coating on his teeth, not bleaching, as I understand it.
No, this is something I have never bothered to do, and shall most likely never do. I have so many allergies and health issues that I do not wish to use any chemicals unless for something which would be lifesaving!!!
